Your Appointment 

New patients are always welcome at Augusta Family Dentistry.  And to make your first visit as relaxing as possible, please arrive at our office approximately 15 minutes before your scheduled visit.  At that time, one of our experienced Patient Care Coordinators will verify your insurance information, will assist you in our required forms (patient information, dental history, dental insurance and notice of privacy practices) and will discuss with you our financial policies and procedures.

When you come to Augusta Family Dentistry for your first visit, please bring:

  • your insurance card
  • any questions you may have regarding your unique dental situation
  • any questions you may have regarding Augusta Family Dentistry
  • your Request for Consultation form from a referring health care professional (if applicable)

If you have dental records at another dental practice, please notify us when you schedule your first appointment.  We will then contact the dental office in order to have your records transferred to Augusta Family Dentistry.  If you are a new patient who has received a Request for Consultation from another health care professional, please indicate that information to us when you schedule your appointment and bring your Request for Consultation form with you for your visit.

In order to make the process even more simple, you may print out the forms that we will be completing with you by selecting Patient Forms.  All patient forms on this website may be completed before your appointment.  However, all forms must be signed by you, the patient, while you are at Augusta Family Dentistry with one of our Patient Care Coordinators.

New patients can schedule their first appointments with us on Mondays through Fridays from 8:30 a.m. to 5 p.m.  Once you are a "patient of record," you are able to take advantage of our extended hours on Tuesdays and Thursdays by scheduling appointments between 8:30 a.m. and 8 p.m.  You are also able to schedule an appointment on most Saturdays from 8 a.m. to 12 p.m.

Before any appointment at Augusta Family Dentistry, always feel free to contact us with questions or concerns.  We are focused on you - on your dental health - and we are dedicated on making your dental health our top priority.

AFD Patients 

If you have an appointment scheduled and are a current patient at Augusta Family Dentistry, be sure to bring your insurance information.  In addition, please inform us of any changes to your billing information, address or insurance when you arrive for your appointment.  We look forward to seeing you!
